Rural Health AI Funding Tracker

The Rural Health Transformation Program (RHTP) is the primary federal mechanism for rural AI investment, distributed through state-level rural health programs. This tracker is for CAH and FQHC leaders who need to understand what is eligible, what is available in their state, and how to apply.

What RHTP Covers

RHTP supports rural hospital transformation, including AI and health technology investments, and is one of the few federal sources that explicitly funds AI readiness work at small rural facilities. Eligible activities typically include readiness assessments, scoped pilots, EHR integration, staff training, and infrastructure.

Five Steps to Access RHTP

  • Contact your State Office of Rural Health.Confirm your facility is eligible and understand the current funding cycle and timeline.
  • Identify your highest-priority AI use case.Focus on the workflow consuming the most staff time or producing the highest error risk, such as documentation, prior authorization, or scheduling.
  • Engage a vendor for a scoped pilot proposal.A written proposal including scope, pricing, and performance benchmarks is typically required for the application.
  • Structure the request around eligible activities.Work with your State Office of Rural Health or grant administrator to map costs into assessment, pilot, training, and infrastructure categories.
  • Establish performance monitoring before the pilot begins.Define success criteria at 90 days, six months, and twelve months. Funders increasingly expect this discipline up front.

Adjacent Funding Sources

HRSA Rural Health Grants

HRSA periodically opens grant opportunities for rural workforce, telehealth, and quality improvement initiatives, several of which can fund AI-adjacent work.

USDA Rural Development

USDA programs support rural broadband and infrastructure, both of which are prerequisites for clinical AI deployment.

State Innovation Grants

Several states have established standalone rural innovation funds with AI eligibility separate from RHTP.

Vendor and Foundation Pilots

Selected AI vendors and national foundations underwrite rural pilots in exchange for outcomes data. Terms must be reviewed against the Vendor Evaluation Guide.
